What is required of a CHAMPS mentor?

Individuals desiring to become CHAMPS mentors are required to

submit an application and are subject to a thorough screening and

background check including references from their Pastor and 2 other

sources. This screening process has been put in place to protect the

well-being of the children served by our program.

Commitment to be a CHAMPS Mentor - Our form,

"Commitment to be a CHAMPS Mentor", outlines the basic

requirements and commitment to be a mentor with CHAMPS.

Would you like to have a Mentor? Mentee Application (All Mentee applications must be accompanied by a Parent/Guardian release form.) Parent Guardian Release Form
